Cancellation Terms*

The membership contract states that monthly memberships are a minimum of a twelve (12) month commitment. This form allows you to get out of your monthly commitment with a fee. Your membership will be canceled upon completion of payment. Your card on file will be charged the cancellation fee. You must fill out this form at least 30 days prior to your renewal date.

I understand that if I am within my first twelve (12) months of my membership contract, I will be charged a cancellation fee. For individual memberships, the fee is $87.00. For family memberships, the fee is $177.00. I understand that this fee will be charged to the credit or debit card on file used for the membership.
